All Tree Maintenance Permit Applications must be approved prior to any work starting.
 
 
 
** $50.00 Permit Application Fee (Required) 
 
** It is the responsibility of the Owner to ensure any Contractor hired has proper credentials, General Liability Insurance and     Workman’s Compensation Insurance. Please provide copies of the required forms.
 
** If approved, all brush, limbs, and/or other debris is to be removed at the Owner’s expense.
 
** Restoring BVTA Limited Common Property, damaged or disturbed, is the sole responsibility of the Contractor and the         Owner.
 
** Any costs incurred to the Bella Vista Townhouse Association, that is a result of the work listed above not being completed     properly, will be assessed to the Owner’s account.
 
** If a BVTA Owner or one of it's residents removes or trims a tree located on BVTA Limited Common Property, without the         approval of BVTA management, they will be fined $250.00, per tree.
